Membership fees are due the day of joining the program. Fees are for an annual membership that begins on the day of enrollment and expires one year from the date of enrollment.

Your dental benefits are honored only at Bell Dental. This is a discount, not an insurance plan. These benefits are not valid at other dental offices. Your dental savings plan cannot be used with another dental plan or combined with any office promotion.

Worker’s compensation, referrals to specialists, hospitalization, hospital charges, and dental costs covered under medical, or auto insurance are excluded. Fees are non-refundable. Premiums or refunds will not be issued at any time if the participant decides not to use their savings plan. The patient’s responsibility for dental costs is due at the time of service.
